Ease in access is vital for the referring office—The Pain Management Group makes it is as easy as 1-2-3! It’s your choice—THREE convenient ways to refer your patients:

- TELEPHONE: call us at 615-941-8527 or 615-941-8525
- FAX: fax your order to 615-941-8522 or 615-941-8515
REFERRAL QUICK TIPS AND INFORMATION:
- Any time you are able to send recent office notes or any imaging/surgical history—we appreciate it!
- We attempt to contact your patient three (3) times with the info on your referral order—thank you!
- Once your patient is scheduled, you will receive a confirmation notification from us with the date/time/location and provider appointment.
- If we are unable to reach your patient, we contact you, keeping you updated. As a courtesy, we are able to assign an appointment for your patient and mail the appointment to the address you have provided—our goal to make it easy for you!
