Rescheduling or canceling

Please let us know as soon as possible so we can offer your time to another patient.

Your first appointment

Please bring any pertinent medical records, such as recent x-rays, MRIs or other imaging studies and reports, along with a list of your current medications.

Depending on your insurance, a referral may be required. If so, your primary care physician must send it to us before your first visit.

As a courtesy, we will file your insurance claim for you, and we make every effort to verify your benefits before your first visit. You are responsible for your account, including any balance your insurance company does not pay. Additionally, if you are a self-pay patient, you must pay with a cashier’s check or credit card only; no cash will be accepted.

At check-in, please have

  • Current insurance card(s)
  • Valid photo ID (driver’s license or other ID)
  • Referral form, if your insurance requires one
  • Co-payment required by your insurance
  • Medical records and imaging (MRI, CT, x-rays)
  • A list of the medications you take

Patient forms

Print, complete and bring these to your first visit.

Safe, responsible care

Opioid Safety

Opioid pain medicine can be an important part of a treatment plan when it is used exactly as prescribed. It also carries serious risks, including dependence and overdose. Our one-page guide explains how to take, store and dispose of your medication safely, and what to do in an emergency.

Take as prescribed

Take only your prescribed dose, on schedule, and never share your medicine. Don’t combine it with alcohol, sleep aids or anti-anxiety medicines such as Xanax or Valium unless your provider approves.

Store & dispose safely

Keep your medicine locked up and out of reach of children, teens and guests. Take unused medicine to a drug take-back box; Tennessee has them in every county.

Know the warning signs

Signs of an overdose include being hard to wake, slow or shallow breathing, blue or gray lips or nails, and pinpoint pupils. Call 911 and give naloxone (Narcan) if you have it.
